Very, very close, within millimeters, but the exact distance varies with the person.

Quite possibly torn if the hymen was indeed still intact. Not all hymens are torn during intercourse only. Very active females often stretch the hymen, eventually tearing it with gymnastics, horseback riding, use of tampons and such, this is why you cant determine virginity by checking the hymen.
