Take the antibiotic you were given. I won't hurt anything and it may actually help with the pain if there is infection (which is probable because of the low-grade fever). If you are not allergic to ibuprofen, you can take 600 mg (usually 3 tablets in OTC form, i.e., Advil, Motrin, etc.) every 6 hours. Take it with food. It will help with the back pain and soreness, together with the antibiotic.

Possibilities are--endometriosis, ovarian cyst, fibroids--with the latter two being more likely. A transvaginal ultrasound will tell you if you have either of these. Do not get the abdominal ultrasound because many times the technician misses things and you have to have a full bladder, which makes the procedure uncomfortable. A vaginal ultrasound is much more definitive. It does not hurt whatsoever to have one done. If you do not have a regular GYN doctor, check with Planned Parenthood again. They may have to refer to you someone for the ultrasound. Sometimes there are GYN doctors that volunteer at free clinics that may be available. If not, the ultrasound will run around $200-300 depending on where it is done (hospital or office).
